Innovative AI tech empowering management of chronic conditions at Arab Health

A UK healthtech start-up using machine learning and clinical innovation is launching a novel proactive monitoring system designed to empower patients with chronic conditions at Arab Health 2022 (24 – 27 January 2022).

The innovative cloud-based software has been developed to track the conditions a patient has, the medications they take and the tests they have done with the aim to improve the management of chronic conditions, enabling it to move from a reactive, to a proactive process informed by data.

Built by healthcare professionals, the software allows GPs and healthcare assistants (HCAs) to make informed decisions, automate key elements of disease monitoring, and ensure patients receive effective care. Thus, it reduces clinical workload, as well as reducing administrative work needed to complete monitoring. For patients, the unique technology helps to improve health, reducing death, disability and hospital admissions resulting from delayed and missed monitoring.

Used by NHS primary care health centres, the system has been shown to improve the care delivered to patients whilst reducing costs to the health system. The Abtrace team has spent three years building state-of-the-art algorithms and connecting into existing healthcare systems to achieve UKCA and CE mark for the product as a certified medical device.

 Abtrace’s new software has been developed as an intelligent platform on top of Electronic Health Records systems enabling actions based on an individual’s medical history in the context of a wider pool of data points validated against NHS approved clinical guidelines. The product has been growing fast, with the company working with over 75 centres in the UK. The technology is expected to impact 1 million patients in the UK by 2022.
